Survey Questionnaires

Created or collected in the Bureau of Research and Statistics.

Consists of completed questionnaires, titled “EXPENDITURES OF FAMILIES OF WAGE EARNERS AND LOWER-SALARIED CLERICAL WORKERS IN HONOLULU, T.H.” A few questionnaires have amplifying information appended. The questionnaires are from two separate surveys. One was conducted in Honolulu in 1943. The 43 questionnaires from that survey have the U.S. Department of Labor, Bureau of Labor Statistics (USDOL/BLS) letterhead. The other survey, despite having the same title on its questionnaires, was conducted on the neighbor islands in 1945. Its 69 questionnaires have the Territory of Hawaii Department of Labor and Industrial Relations, Bureau of Research and Statistics (DLIR/BR&S) letterhead. The 28-page questionnaires contain raw demographic and economic data, such as family size, ages, ethnicity, employment, income, and expenses for various items of food and clothing. The data were analyzed and statistics were compiled. The results of the 1943 survey were published by the USDOL/BLS, in the April 1944, issue of their “Monthly Labor Review.” This serial is available at several libraries in the University of Hawaii system. The results of the 1945 survey were published by the DLIR/BR&S, as its Bulletin no. 18, Family Income and Expenditures on the islands of Hawaii-Maui-Kauai, 1944. Rulings

Rulings excerpted from the minutes of the Honolulu board interpret the law and its operations, and cover such matters as determination of average weekly wage; coverage for employees of sub-contractors; required forms of policies; lump sum payments; etc. Includes sample forms referred to in the rulings.

City & County of Honolulu Industrial Accident Board

Reports

Limited to the Hawaii County board. Monthly reports from 1925 to 1929 from the secretary to the board list totals of first and final reports, lost work days and fatalities, brief information on particular claims and a list of monthly bills.
The annual, 1915-1928 and fiscal July to June reports, 1915-1928, in letter form to the Governor, provide statistical summaries of the year's operations with number of accidents, fatalities, lost work days, compensation payments, etc. There is a brief discussion of cases of note. Administrative matters such as operating expenses are also covered.
The ten-year report for 1915 to 1925 summarizes operations and comments on the board's experience with employers and developments in the law during this time. Attached are statistical exhibits on accidents, nationality of employees, lost workdays and compensation paid.

County of Hawaii Industrial Accident Board

Records of the State Comprehensive Employment and Training Program

Created or compiled in the Office of Manpower Planning, the original name of the Office of Employment and Training Administration, DLIR.

Contains correspondence, memoranda, notes, procedures and guidelines, reports and evaluations, statistical tables, minutes, brochures, contracts, press releases, a slide presentation, and program forms with instructions. Documents the origins, evolution, administration and termination of the SCET Program, including information on the purposes and attainments of specific projects funded by the program.

Records Concerning Delinquent Employers

Records limited to Honolulu board. Contains files on delinquent employers, i.e., those who failed to obtain or renew policies or provide alternate security. Typical files include delinquent employer cards with summary case information, notice letters, subpoenas and transcripts of board hearings. Unresolved cases were referred to the City and County Attorney and include copies of further legal proceedings. The files are not complete, having been sampled during initial processing in the late 1980's, but include samples of various types of employers including taxi companies, dance halls, newspaper distributors, building contractors and various small businessmen such as grocers, launderers, plumbers, etc.

Outgoing Letters of the Bureau of Workmen's Compensation

Outgoing letters of the county agents for Hawaii, Maui and Kauai, copied to the Bureau, and correspondence with the Bureau. The letters cover routine operations in each county, under the guidance and review of the bureau. Most of the letters are to employers and include such matters as inquiries on reports due, errors in benefit calculations, notices of hearings and delinquencies on security and inquiries on specific cases. Correspondence with the bureau includes questions and directives on administration of the law in general and on non-routine cases.
There are also monthly statistical reports and some blank forms. Kauai and Maui correspondence runs through 1941 only.

Minutes of the Employment and Training Councils

Created by the State Manpower Services Council, its successor the State Employment and Training Council, or by the county manpower planning councils or their successors, the county employment and training planning councils of the neighbor island counties. Collected in the Office of Employment and Training Administration, DLIR, or its predecessor, the Office of Manpower Planning.

Contains agendas and minutes, as well as attachments to the minutes, such as by-laws, correspondence, notes, memoranda, comments, statistical program summaries and reports. Documents the origin, composition and operations of the councils. Includes information on programs administered pursuant to the federal Comprehensive Employment and Training Act (CETA) and the State Comprehensive Employment and Training (SCET) program.

Minutes

The minutes of the meetings of the Honolulu board run from its inception in 1915 through 1939, when its functions were taken over by the Bureau of Workmen's Compensation. Regular weekly hearings of the Bureau are included from 1940 through 1941. There is a six-month gap in 1917 when the board's activities were suspended pending a court ruling on the constitutionality of the law.
These records are contained in the 13 bound volumes, comprising 31 linear inches, and include fairly detailed summaries of claims before the boards, their disposition and awards, and names of parties present; rulings on the scope and extent of the law; definitions and interpretations of the law; procedural guidelines; rulings on forms of insurance policies; actions taken on self-insurance applications and delinquencies; entries on protests and appeals; notes on opinions solicited from the City & County attorney; notes on correspondence; early news clippings; some statistical data; and internal business matters, such as personnel and funding, etc. Records created after the functions of the Honolulu board were taken over by the Workmen’s Compensation Bureau are less detailed. Also includes copies of the first and second annual reports bound in vol. 1 for the six months ending Dec. 1915 and year ending 1916.

The minutes of the meetings of the Hawaii County board include notes on all cases and hearings, parties present, and issues, facts and findings. Also includes notations on self-insurance applications and delinquent employers. Other matters noted include correspondence and discussion on procedural inquiries and administration. Monthly vouchers and accounts payable are listed. These records are much less detailed than the Honolulu board minutes and are contained in one folder of .75 linear inches.

Legislative Records

Contains the Honolulu board's draft bills for proposed changes to Act 221 along with correspondence, research memos, surveys, reports and recommendations. Subject areas include second injury and rehabilitation programs, broadening of coverage and extension of benefits to employees not previously covered and changing the definition of dependents.
